How to get a facility to TB Phase 4?

A facility has reached TB Phase 4 when the relevant THIS Key Implementers (TKIs) have completed the “Final Checklist before TB Module Go Live from Implementation Guide” (available under Resources & Tools in the ‘Support Tools’ folder), and have activated the TB module, and when the facility admin clerks have commenced the initial capture.
